Every form a patient completes is kept on their record, with the wording they were shown, their answers, the acknowledgement and their signature. You can also see which forms are still waiting.
Before you start
- Any member of the team can view a patient’s forms: owners, practitioners and receptionists.
Steps
- Go to Patients and open the patient’s record.
- Select the Forms tab. Each form shows its title, type (Consent, Questionnaire or Aftercare), where it was completed (in clinic or remote) and the date.
- Check the badge on each form:
- Done: completed. Select the form to open it.
- Pending: sent but not completed yet. Pending forms can’t be opened.
- The completed form shows:
- the title, type, and the date and time it was completed,
- in clinic, with [team member] or completed remotely,
- the wording and each question with the patient’s answer. An unanswered question shows Not answered,
- Acknowledged. with the statement the patient ticked, on consent and aftercare forms,
- the patient’s Signature, if the form required one.
What happens next
Nothing changes when you view a form. Completed forms can’t be edited, by you or the patient. If something needs correcting, ask the patient to complete the form again.
Good to know
- The form shows the exact version the patient completed, even if you’ve published a newer version since. See Publishing forms and versions.
- Answers are decrypted only when you open a form. If a form’s answers can’t be shown, a message says so; the wording, acknowledgement and signature are still displayed. Please report this to support.
- To chase a pending form for an upcoming appointment, open the booking and select Send forms again. See Send forms automatically.
- Patients can reopen their own completed forms from Completed on the Your forms page in the portal.
