Deactivate or delete a form
- Owner
On this page
When you stop offering a treatment or replace a form, you can deactivate it or, if it has never been used, delete it. Signed copies are protected either way: a form a patient has completed can’t be deleted.
Before you start
- You need to be the clinic owner.
- Decide which you need:
- Deactivate keeps the form and its history but stops it being offered. You can activate it again later.
- Delete removes the form permanently. It’s only available for a form that has never been used and isn’t attached to an appointment type.
Deactivate a form
- From the account menu at the bottom of the sidebar, go to Clinic settings > Clinic > Forms.
- On the Live forms tab, find the form and select Deactivate. You can also select Deactivate on the form’s own page.
The form now shows an Inactive badge. It stops being sent to patients straight away, including from appointment types it’s attached to, as aftercare, and from Send forms again. It’s also no longer offered when you attach forms to an appointment type or complete a form in clinic. You don’t need to untick it on each appointment type.
To use the form again, select Activate. Publishing a new version also makes it active again.
Delete a form
- First remove the form from any appointment types it’s attached to. An inactive form doesn’t appear in Forms for [appointment type], so activate it before you untick it.
- Go to Clinic settings > Clinic > Forms.
- On the Live forms tab, select Delete on the form’s row. If there’s no Delete option, the form has been used or is still attached somewhere.
- Select Delete form to confirm.
What happens next
A deleted form and all its versions are removed and can’t be recovered. A deactivated form stays in your Live forms list, greyed out, with its version history intact.
Good to know
- Once any patient has been sent a form, it can’t be deleted, even if they never completed it. You’ll see “This form has already been used, so it can’t be deleted. Deactivate it instead.”
- If the form is still attached to an appointment type, you’ll see “This form is attached to an appointment type. Remove it there first, or deactivate it instead.”
- Deactivating or deleting a form never changes forms patients have already completed.
- Form templates can’t be deleted or deactivated. They’re only copied into your live forms when you add one.
