Use a form template

Form templates give you ready-made wording for consent forms, medical questionnaires and aftercare forms. You don’t send a template directly: adding one gives your clinic its own copy, which you can use straight away or edit first.
Before you start
- You need to be the clinic owner.
- Read the template before you use it. Templates can include notes for the clinician, shown in a panel headed For the clinic, not the patient, that say what the template covers and what the clinician or prescriber must confirm before using it.
Steps
- From the account menu at the bottom of the sidebar, go to Clinic settings > Clinic > Forms.
- Select the Form templates tab. Templates are grouped into Consent forms, Medical questionnaires and Aftercare forms.
- To find a template, type in Search forms by name or product…. The search also matches the products named in the clinician notes.
- Select a template’s title to read its wording and clinician notes. Templates are read-only.
- Choose how to add it, either on the list or on the template’s own page:
- Add to live adds your copy and publishes it as it is, ready to send. From the template’s page, you’re taken back to your forms.
- Add and edit adds your copy as a draft and opens it in the form builder, so you can change the wording before you publish.
Add several templates at once
- On the Form templates tab, tick the box next to each template you want.
- Select Add [number] to live forms in the bar that appears. Each one is added and published as it is.
If any are skipped, for example because you’ve already added them, Clinicify tells you how many were added and why the rest were skipped.
What happens next
Your copy appears under Live forms. A copy added with Add to live shows a Live badge; one added with Add and edit shows Draft until you publish it. The template now shows Already added on the Form templates tab. Your copy keeps the template’s clinician notes, visible only to staff.
Good to know
- Your copy belongs to your clinic. You can edit, publish new versions of, deactivate or delete it like any form you built yourself.
- Later changes to a template don’t alter a copy you’ve already added. If you want updated wording, add the template again with Add and edit, or edit your copy.
- Consent must be specific to the procedure. Check that the wording matches the treatment you offer before you send it.
- Next, attach the form to the right appointment types: see Link forms to appointment types.
