Consent that stands up months later

Paper forms get lost and PDFs come back half-finished. Clinicify sends the right forms automatically before each appointment, collects a signature, and keeps a record of the exact wording the patient agreed to.

Sent on Sunday, signed on Sunday, against the version they actually saw.
The real problem

A consent form feels like something you do on the day. It is not. Its value arrives months later, when somebody asks what the patient was told.

On that day the signature is the easy part. What matters is showing the exact words that were on the page in front of them, on the date they signed.

A form you have edited since cannot do that.

What changes

  • Forms arrive before the patient does

Consent and medical questionnaires go out when the booking is made, or a set number of days before the appointment, without anybody remembering to send them.

  • You can show exactly what was signed

Every published version is kept, so a signature points at the wording that patient saw rather than at whatever the form says today.

  • Aftercare goes out on its own

Mark the appointment complete and the aftercare is sent. The patient has it the same day, and it stays on their record for when they lose the email.

A normal day

Without
  • Tuesday: a clipboard and a biro in reception
  • Wednesday: chasing a questionnaire that came back blank
  • November: which version of the consent form was that
With Clinicify
  • Tuesday: signed the night before
  • Wednesday: it arrived complete or not at all
  • November: that version, signed on that date, on the record

If there is ever a complaint

Published versions cannot be changed. An edit creates a new version, so a signed record never drifts underneath you. Signatures are required on consent forms and questionnaires by default.

You do not start from a blank page either. Templates for consent, questionnaires and aftercare are there to use as they are or adapt to your clinic.

The detail, briefly

  • A form library and builder, with drafts and published versions
  • A published version cannot be edited; an edit makes a new one
  • Forms combine body text, questions, an acknowledgement and a signature
  • Forms are linked to appointment types
  • Automation on booking, a set number of days before, and aftercare on completion
  • Patients complete forms in the portal; staff can complete them in clinic
  • Templates for consent, questionnaires and aftercare, with guidance notes only practitioners see
  • Answers are encrypted at rest
Form requests go out by email. There is no SMS delivery for forms.
