If a patient arrives without completing their forms, or you need a form signed on the day, you can open it on a clinic device and hand it to them. The completed form is saved to their record in the same way as one completed in the portal.

Before you start

  • Any member of the team can do this: owners, practitioners and receptionists.
  • The form must be one of your clinic’s Live forms, published and active. Form templates and deactivated forms aren’t offered: add a template as your own copy first (see Use a form template), or activate the form again.
  • The patient needs to be on your patient list. See Add a patient.

Steps

  1. Go to Patients and open the patient’s record.
  2. Select the Forms tab.
  3. Select Complete in clinic.
  4. In Complete a form in clinic, choose the form. Only your clinic’s own published, active forms are listed. It opens on its own page with the form’s title, wording and questions.
  5. Hand the device to the patient. They:
    • answer the questions (required questions must be answered),
    • tick the acknowledgement box, on consent and aftercare forms,
    • sign in the Signature box with a finger or mouse, if the form needs a signature. Clear lets them start the signature again.
  6. The patient selects Submit.

If something is missing, the form says what: for example “Please answer all required questions.”, “Please tick the box to confirm.” or “Please add your signature.”

What happens next

The form shows “Form submitted” and returns you to the patient’s record. On the Forms tab it appears as Done, marked in clinic, with the date. When you open it, it also shows which team member was with the patient.

Good to know

  • To leave without completing, select Back to patient. Nothing is saved until the patient submits.
  • The form is saved against the version that’s published at that moment.
  • If a form is deactivated after you open it, the page shows “This form is deactivated, so it can’t be completed.”
  • An in-clinic form isn’t linked to a particular appointment, and it doesn’t replace a form already waiting in the patient’s portal. If the patient had one outstanding, it stays under To complete in their portal.
  • Signatures are saved as an image with the form. Answers are stored encrypted.
