Forms let you collect consent, medical history and aftercare acknowledgements from patients without paper. Patients complete them in the patient portal before or after their appointment, or on a device in your clinic.

The three types of form

TypeWhat it’s forSignature
Consent formWording the patient reads and agrees to before a procedureDrawn signature required by default
Medical questionnaireQuestions the patient answers, such as medical historyDrawn signature required by default
Aftercare formAdvice the patient reads and acknowledges after treatmentNo signature by default

Consent and aftercare forms end with an acknowledgement statement the patient ticks. You can turn the drawn signature on or off for any form.

Live forms and form templates

Your forms are managed in Clinic settings > Clinic > Forms, which has two tabs:

  • Live forms are your clinic’s own forms. Only these can be sent to patients or attached to appointment types.
  • Form templates are ready-made wording you can adopt. A template is never sent as it is. When you add one, you get your own copy to keep, edit and publish. Later changes to the template don’t alter a copy you already have.

You can also start a form from scratch with Blank form.

Drafts, publishing and versions

Every form is edited as a draft. Patients only ever see a published version. When you change a published form, you edit a new version and publish it; the old version is kept, and anything a patient already signed stays linked to the exact wording they saw. See Publishing forms and versions.

How forms reach patients

  1. You attach forms to an appointment type, or mark a form to go with every new booking.
  2. Your Form automation settings decide when they’re sent: when the booking is made, a set number of days before, or (for aftercare) after the appointment.
  3. The patient gets an email asking them to complete their forms in the portal. The email gives a count of forms, never their titles, so it doesn’t reveal the treatment.

If a patient is with you in person, any member of your team can open a form on a clinic device for them to complete and sign.

Where completed forms are kept

Completed forms appear on the Forms tab of the patient’s record, showing the wording the patient was shown, their answers, the acknowledgement and their signature. Answers are stored encrypted. Patients can reopen their own completed forms in the portal.
